Where should we take our children for medical care after June 26, 2025?

If your insurance allows, we recommend that you schedule with University of Utah pediatrics.
They have a number of convenient locations in the Salt Lake Valley, and we have been coordi-
nating with them to make sure they are ready to take Sunnyside patients starting in July. We are
entrusting the storage of our medical records to their system, so providers within the University
of Utah system should be able to access your old records easily.
If your insurance is through Select Health our website now includes a list of alternative clinics
and providers, many of whom accept Select Health patients.

What about sports, camp, school forms and prescription refills?

Please get us any forms by June 20. Remember that if it has been over a year since the last
physical, we will need to have an appointment prior to completing any forms.
If we are prescribing any ongoing medicine for your children, please take time now to check the
status of their refills. If your child has not been seen in more than 6 months it is likely they need
an appointment! If you are going to need a refill before your child's next "well child visit" please
get in touch by June 20.
Please do not wait until the last minute to request forms or refills. If your child needs an appoint-
ment, we would like to plan that while there are plenty of openings on the schedule for you to
choose from.

What about medical records?

Your child's medical records can be found on the patient portal. You can print these records (in-
cluding vaccine records) from home. Please contact us if you need help setting up a portal ac-
count. The portal will be active through the end of July, 2025.
The records will be stored long-term through the University of Utah. If you are planning to have
your child seen within the University of Utah system, they will be able to access records once
you have given written permission.
If you would like medical records sent to a different office, please let us know before June 20.
Keep in mind that for most healthy children your new provider may only want to review
the vaccine records and the growth chart.
You can print a records release form from our website (https://www.sunnysidepeds.com/) and
email a scanned copy to us at medrecords@sunnysidepeds.com.
Because of the transition, requests made after that June 20 will need to be made through the
University of Utah hospital system. Details about this will be available on our website at a later date.

It’s bittersweet but after almost 28 years of practicing pediatrics downtown, I will be retiring. It’s been an honor to share this amazing and rewarding job of caring for kids with each of your families! 

I plan to be reading, volunteering in the community and hiking the beautiful trails of the Mountain West!
